New Post-Concussion Return-to-Play Requirement: We'll need the written clearance from an MD, DO, PA or NP, signature and we also require both students and their parents/guardians before the student can return to activity. It is also required the Return to Activity & Post Concussion Consent Form be emailed or faxed to the MHSAA and kept on file at the school for seven years following a student's high school graduation. The same form signed by the medical professional must then be signed by the student and parents before the student may return to activity. Students who participate without the required approval are ineligible athletes. The form is on the MHSAA website - Health and Safety - Return to Activity & Post-Concussion Consent Form.
